Three shifts to find a 0.003 inch offset in a worn fixture pin

Ran a job all day Saturday and every part came out 3 thou off on the same hole, turned out the locating pin had a flat spot worn on one side and I wasted 2 hours checking tool offsets before I grabbed a mic and measured the fixture itself, has anyone else had a dumb wear spot hide like that?
