Ground rod depth, I was doing it wrong for 8 years
I was getting failing grounds on new builds around Spokane for years. Always blamed the soil, never thought to check the rod itself. Then one inspector made me pull a rod after I pounded it in. The damn thing was only sunk 6 feet because my sledge technique was bouncing it sideways, not driving it straight. He showed me a 4 foot ruler and it all clicked. Anyone else had an inspector catch a basic habit you'd been repeating since your first year?

Wait, eight YEARS?? And nobody caught that until now? That's wild. I would've been so mad at myself, but also lowkey impressed the inspector figured it out.
