Was at the DMV in Phoenix and my paperwork just vanished
I had all my documents in a manila folder and somewhere between the counter and my car they disappeared. Spent 45 minutes retracing my steps through that whole building before a clerk found it wedged behind a vending machine. How do you keep track of important papers when you're in a hurry?

Had a friend who lost his truck registration at the DMV once. He'd set it on the counter to sign something and it got scooped up by a stack of papers an employee was moving. They found it three weeks later in a filing cabinet in a back office. They mailed it to his house with a note that said "found this in our files." He still has no idea how it got there. Now he keeps everything clipped to his shirt pocket with one of those retractable badge holders. Looks ridiculous but he hasn't lost anything since.

Did you check if the folder slid under something? I keep all my DMV papers in a clear plastic folder with a zipper now. After losing my birth certificate for a week, I bought one of those bright orange ones. It's harder to miss. I also take a photo of every document before I go inside. That way if something goes missing, at least I have the info. And I always double check my hands before walking away from any counter. Don't shove stuff in your pocket either, that's how it falls out in the parking lot.
