Post tension cable let go on my second pour, lucky nobody was near it

I was tensioning a 20 foot slab in my backyard in Austin last Tuesday and the wedge slipped around 14,000 psi, the cable whipped out of the pocket and snapped a 2x4 clean in half about 3 feet from my leg. I spent the next hour digging out the broken strand and re-sleeving the whole run before re-tensioning with a different chuck and slower pump speed. Has anyone else had a wedge seat fail on a small residential pour, and what did you switch to for safety?

Ever tried a two-piece wedge with a torque wrench? Worked for me on my driveway.
