We just added a quick 5-point check for tool offsets, coolant levels, and program zero before every run, and it basically stopped those dumb first-part mistakes, so has anyone else tried something like this?

I mean that's a great result, but I wouldn't call that a pre-op checklist. That's just a standard setup procedure. A pre-op is more for safety stuff before you even power the machine on.
